Visit Therapist on the trader screen and hand over 250,000 Roubles when the dialogue option appears.

Tip
Pure trader screen. Open Therapist's tab and click through her dialogue — she's the medical/healing trader and Tour starts you off with her because she's the unofficial gatekeeper of Streets. Hand over 250,000 Roubles when the option pops; the cash doesn't need to be Found-In-Raid, just in your stash. If you're broke, sell some Scav-run loot to cover it (electronics, food, fuel sell well to Therapist herself). After paying, Streets of Tarkov unlocks immediately. Don't queue Streets yet — finish more of Tour first to unlock other maps; you'll want options.
